Radioactive Release at New Mexico Site Called 'Preventable'

A radioactive release above ground during a February accident at an underground federal nuclear-waste repository in New Mexico was preventable and the result of management's "failure to fully understand" and control the risks at the site, according to an Energy Department report issued Thursday.
